doing business as Dance With Joy Enterprises, Inc., located at 1544 Crow Mountain Road,

Russellville, Arkansas.

The annual enrollment fee is $25.00 person/$30.00 family. This agreement is entered into with the

full understanding that it is a binding obligation of monthly payment of $ ________________ for the

period of ______________________, 20____ to ____________________________,20_____,

regardless of attendance. Post dated checks will be deposited on the 1st of each month. 

A $25.00 fee will be charged on all insufficient payments.

The attached Rules & Regulations are a part of this agreement and they have been reviewed by the responsible party and/or discussed with the child/children involved in this instruction. Having received these Rules & Regulations and full understanding all procedures and safety precautions. Joy Murphy and Dance With Joy Enterprises Inc., is hereby released from any and all liability, claim, expense or loss unless caused by their negligence of improper conduct and I/we hereby accept full responsibility under this contract.
